We want to provide additional information about procedural changes that we have in place to help maintain safety and continue providing care due to the coronavirus.
For patients and providers who are continuing to meet in the office, we have made several changes to facilitate social distancing and maintain a clean, healthy environment.
- Patients are encouraged to return to their vehicle after check in to wait on their clinician. Please confirm your cell phone number at check in and your clinician will either call you or step outside the building to bring you directly to your appointment.
- Commonly used surfaces and areas will be cleaned using appropriate sanitizing agents on a frequent basis throughout the day.
- Patients who develop a fever or any respiratory symptoms prior to the appointment are asked to call and reschedule. Any fees associated with a late cancellation will be waived in this instance.
- Any additional individuals who accompany a patient to an appointment are encouraged to wait in their vehicle as opposed to the common waiting area.
Telemental Health Availability:
Our providers will be able to provide telemental health sessions for ongoing patients for the duration of this situation. Specific procedural information can be found in the document below. Patients are asked to complete this packet and return it to the office in order to access these services. You may call the office either prior to or after your scheduled session to pay any co-pay amount. If you are not able to reach the office that day, one of our staff will reach out to you within 48 hours of your appointment to assist you with this.
Telemental Health Forms & Instructions:
When completing the forms, please make sure that you fully complete the release of information. This release is only to allow for information to be shared due to an active medical emergency that could take place during a telemental health appointment. It does not allow your information to be shared in any other context separate from that.
When completed, these forms can be returned to our office during standard operating hours, submitted by fax at (706) 653-7843, returned by mail, or submitted as an e-mail attachment to our business office at email@example.com This e-mail may only be used for telemental health online form submission and cannot be used to request or change appointments or for any clinical communication.
